Incident Management, Reporting and Records

As per UniSQ Incident Management Procedure, all incidents, injuries, hazards, and near misses must be reported immediately to:
 • Your Contract engager/ Project Officer
 • UniSQ’s Health, Safety and Wellbeing (HSW) Team via email unisqsafety@unisq.edu.au 

Immediate Response
 • Prioritise the physical and psychological safety of all individuals.
 • Provide first aid or contact emergency services (000) if required.
 • Contact UniSQ Security on 07 4631 2222 for urgent support. 

In the case of a Notifiable Incident (e.g. serious injury, illness, or dangerous incident), the scene must not be disturbed after emergency response actions, until directed by the HSW Team.

Reporting
All incidents must be reported in Safetrak, UniSQ’s incident management system, as soon as reasonably practicable by the affected person, contract engager/ project manager or the HSW Team.

Records
UniSQ will maintain records of all contractor inductions, training, licenses, and incident reports. Contract Engagers/ Project officers are responsible for ensuring:
 • Induction records are stored securely
 • Licenses and certifications are verified and logged
 • Incident reports are submitted to Safetrak and retained for audit purposes.
